Texas Health Care Solutions is a locally owned and operated licensed and certified home health agency that  is dedicated to providing professional personal health care services to individuals in their homes, apartments or assisted living communities. Our home nursing care and rehabilitation services are designed to promote  wellness, safety, and independence. We provide registered nurses, home health aides, personal care providers, physical therapists, occupational  therapists, home health aides, and medical social workers.

Skilled Nursing Care

Our registered nurses (RNs) provide skilled nursing care that reflects your  medical needs. An individualized health care plan is developed for each  Texas Health Care Solutions client. This plan is based on a personal health care  evaluation and includes home nursing care instructions. Skilled nursing care is  a vital component of our Hill Country home health care agency.

Rehabilitation

Texas Health Care Solutions offers a wide variety of rehabilitation services  in your home. Our agency provides physical therapists and occupational therapists. Rehabilitation is a  very important part of your personal health care services.

Our quality  assurance staff routinely monitors the plan of care to ensure consistency,  quality and appropriateness of care.

Credentials of Staff:

Every state has its own licensure and continuing  education requirements for each health care specialty. In addition to these  stringent requirements, Texas Health Care Solutions enforces its own standards of  professional development. Our inservice education program provides a  comprehensive orientation, skills check-off, and regular continuing education  programs to ensure that employees of all disciplines remain current regarding  new procedures, treatments, equipment and resources.

Patient Care  Management:

Texas Health Care Solutions offers state-of-the-art technology to  develop and monitor a comprehensive plan of care for each patient. Utilizing a  home care management information system, the patient's medical history and plan  of care are available to the referring physician and all members of the health  care team. Patient status, doctor's orders, medications, clinical findings,  goals and discharge plans are updated and reviewed online for immediate access  by the agency's clinical staff. A comprehensive drug database, which covers more  than 60,000 drug products provides information on drug interactions, side  effects, proper usage, special instructions and precautions. Texas Health Care Solutions’s  investment in computer technology allows for coordination of patient assignments  based on consistency of staff and geographic location. The accumulation of  statistical data on population demographics, costs of service by diagnosis and  patient outcomes allows the clinical staff and management to deliver cost  effective quality care.

Referral Process and Payment Source

A Texas Health Care Solutions representative is available to answer your  questions Monday-Friday 8:00 a.m. to 5:00 p.m.CST. A complimentary on-site  assessment can be conducted by a Kerrville Home Heatlhcare "Care Coordinator" for clients who are in the hospital or a health care facility.

Medicare and many insurance companies cover the cost of  home health services  when specific criteria are met. All homecare services are also available on a  private pay basis.
